Frequently Asked Questions

  • Is a doctors referral needed?

    • In the state of Colorado, physical therapists have direct access and can provide evaluation and ​treatment without a physician's referral.  For all Medicare patients, a referral is preferred, but not required and a signed plan of care will be accepted.  Evaluation notes and plan of care will be sent to your physician upon request for non-Medicare patients. 

  • What should I expect from my first new patient visit?​

    • You should expect to spend 50-60 minutes to discuss your current complaint, past medical history, evaluation and treatment will be provided.  Recommend wearing comfortable clothes and brining shorts or a tank top depending on body location being treated. ​

    • For all pelvic floor patients, expect internal pelvic floor examination. 

    • You may feel sore if you are having dry needling or manual therapy done.  This is normal, drink a lot of water to flush out toxins.  If you have any severe exacerbation of symptoms let us know by email or phone. 

  • Do you take commercial health insurance?​

    • Roots Physical Therapy is not in network with commercial health carriers.  We can provide a superbill for your services which you can submit to your insurance company to request reimbursement. 

  • What If I have Medicare?​

    • We will need a copy of your driver's license, Medicare insurance card, list of any history, surgeries, and medications you currently take. 

  • How many visits will I need?​

    • Depending on the initial evaluation, Dr. Pavlantos will determine the recommended number of visits and discuss with you at initial treatment.  For certain patients, 30 min sessions & packages will be available, but more complex patients may require one hour treatment sessions to provide the best care. ​

  • Is there a cancellation fee?​

    • A cancellation fee of $75 will be applied to any cancellations made sooner than 24 hours in advance of your appointment.  Late cancellations do not allow for other patients to schedule. 

    • New Eval No show/late cancel will be charged the full rate of the visit.  

  • What is concierge physical therapy?​
